A qualified Life Status Event is an event that allows you to change your benefit coverage during the plan year including:
- Marriage
- Birth or adoption of a child(ren)
- Death of a covered dependent
- Divorce or legal separation
- Dissolution of domestic partnership
- Spouse or eligible dependent begins employment
- Spouse, domestic partner, or eligible dependent loses coverage
- Dependent child reaches the age limit for coverage
If you experience one of these life status changes, you may make a change to your benefit consistent with the change. For example, if you get married or have a child, you can add that eligible dependent to your coverage.
To request changes, go to Dayforce to complete the “Life Event Declaration Form” (Benefits > Forms):
- You must provide documentation to support the type of qualifying change, as well as the effective date.
- You must also complete the Desjardins enrollment form and submit it to Desjardins.
