Frequently Asked Questions

You have 30 days from your date of hire to elect your benefit coverages.

You must complete your enrollment through Dayforce (Opens in new tab) and complete and fax the application (found within the Dayforce enrollment) to Desjardins.

For more detailed instructions on how to enroll, please utilize the My Path function in Dayforce:

  • Click on the “help” icon in the upper right corner of your employee home screen.
  • Select “Employee Training” > “Benefits” > “Selecting your benefits”

Please note: Dependents must be added prior to beginning your enrollment.

Your medical, vision, and dental benefits are effective on the first of the month following 90 consecutive days of employment.

Minimum weekly hours requirements:

  • 21 hours per week Perth Union & Perth Clerical (non-union).
  • 26 hours per week all others.

LTD coverage is effective on the 120th consecutive day of employment. Dental coverage for Perth Union Associates is effective after 12 months of service, per the contract.

To change you benefit elections, you must experience a qualified Life Status Event. You have 30 days from the date of the LSE to declare your event in Dayforce, provide documentation, and submit your changes.

Events that allow you to change your coverage during the plan year are: marriage; birth or adoption of a child(ren); death of a covered dependent; divorce or legal separation; dissolution of domestic partnership; spouse or eligible dependent begins employment; spouse, domestic partner, or eligible dependent loses coverage; or dependent child reaches the age limit for coverage (“Who qualifies as a dependent?“). If you do not complete this process in 30 days, you will have to wait until Open Enrollment to make plan changes effect January 1st of the next plan year.

LSEs are declared through Dayforce (Opens in new tab). Select “Benefits” > “Forms” > “Life Event Declaration.” You must provide documentation to support the type of qualifying change, as well as the effective date.

You must also complete the Desjardin enrollment form (English (Opens in new tab) | French (Opens in new tab)) and submit it to Desjardins.

Caleres will offer an Open Enrollment period in November for associates to make enrollment changes that will be effective on January 1st of the following year.

Since the beginning of the pandemic, printing and sending of insurance certificates for both new contracts and ongoing contracts has been suspended.

Information related to group insurance, such as the list of insurance benefits and the member payment card, is available on the Desjardins secure plan member site or the Omni mobile app. The secure site and mobile application also allow members to make most of their transactions online and offer direct deposit to accelerate benefit payments. HOW CAN I REGISTER FOR OMNI, THE DESJARDINS MOBILE APP?

Please use the Desjardins secure member site (Opens in new tab) to download the mobile application Omni, which provides access to the payment card, claims and direct deposit.

There is no communication sent by Desjardins once you are enrolled. Please utilize the Desjardins secure site and mobile app to verify your coverage.
